Back-to-School Health Checklist for Families
Start the school year prepared with a simple checklist for physicals, vaccine records, medications, sleep, and healthy routines.

The weeks before school starts are a useful time to gather health records, schedule needed visits, and reset routines. A little preparation can make the first days of school feel much less rushed.
Put records in one place
Keep copies of your child’s vaccine record, school or sports physical form, medication list, allergies, and emergency contacts together. Requirements vary by school and activity, so check directly with your child’s school or program.
Plan for the routine, not just the paperwork
- Schedule any requested school, camp, or sports physicals early.
- Ask about vaccines that may be due and bring prior records to the visit.
- Review medication needs, including inhalers, allergy medicines, or action plans.
- Gradually move bedtime and morning routines toward the school schedule.
- Talk with children about handwashing, hydration, and telling an adult when they do not feel well.
If your child has a chronic condition or needs medication during the school day, contact the school nurse or administrator early to confirm the required forms and plan.
